Disability Tax Credit Application

CRA has introduced a digital application for the Disability Tax Credit (DTC).

If you and your medical practitioner think you may be eligible for the DTC, your medical practitioner can now complete Part B of Form T2201 via the digital application at canada.ca/dtc-digital-application.

At the end of the application, a PDF will be generated that your medical practitioner must print and sign. As the applicant, you can then complete the sections in Part A that apply to you.

There are two ways to submit the completed Form T2201: electronically using the "Submit documents" feature in My Account or Represent a Client or by mail to your nearest tax centre.
